The club announced that the pair tested positive for the virus on Boxing Day. Twickenham is expected to host the game on Monday.
A club statement said: “Harlequins opening half Marcus Smith and backrow James Chisholm both tested positive for COVID-19 via a PCR test earlier this week.
“Due to health and wellness regulations, all players must observe a clear and gradual return to match intensity after a positive COVID-19 result before resuming the match.
“Unfortunately, there was not enough time for either player to go through these protocols before Big Game 13, and therefore are not available for selection for this game.
“Both should be ready for the club’s Gallagher Premiership game in Gloucester on January 2.”
Quins also announced that Viliami Taulani will replace Dino Lamb in jersey 20 for the game.
